Simply click above link, print and bring in the registration form to any location or let your host and server know that you want to be a member of the Perfect 10 program with Urban Arts Academy.The manager will bring you a card that you can use anytime.It is helpful to let them know right away as it takes a few minutes to get the information processed.
The membership Number for Urban Arts Academy is 70005.Your server or host will need this number to issue your membership card.
